Referral for service

To find out whether you are eligible for any of Council's services, you first need to make a referral.

The way to make a referral depends on your age and whether you are an existing client of Moreland's Aged and Community Support Service.

Residents can refer themselves or a referral can be made by a relative or friend, doctor or other service provider, with the person's consent. 

New clients under 65 year of age, or over 50 years or under if Aboriginal or Torres Strait Islander

As of 1 March 2018, the National Disability Insurance Scheme (NDIS) is live in Moreland.

If you are under 65 years of age, or are under 50 years of age if Aboriginal or Torres Strait Islander, and are seeking support services, you need to contact the National Disability Insurance Scheme (NDIS) on 1800 036 377 to test for eligibility.

It is expected to take up to 12 months for eligible people with disability to enter the NDIS. During this transition, Moreland City Council may provide short-term funded services for 3 months in certain circumstances while eligibility is being tested.

  • Check for an NDIS information session being held in the Moreland area.

New clients over 65 years of age, or over 50 years of age if Aboriginal or Torres Strait Islander

If you are aged over 65 years, or are over 50 years of age if Aboriginal or Torres Strait Islander, and are not an existing client of a Moreland Aged and Community Service, you must register as a My Aged Care user before you can access programs and support from Moreland City Council. 

My Aged Care is the main entry point to the aged care system in Australia for new clients. It enables older people, their families, and carers to access information on ageing and aged care, have their needs assessed and be supported to find and access services.
